Care Coordination

Care Coordination services provide an experienced registered nurse to provide support in coordinating a wide range of health and community services to enable a client to continue living at home. Care coordination services take an holistic approach and tailor a service mix to the needs of each client. The services coordinated by KinCare may include direct services from KinCare and a broad range of other services from other service providers.

The following table highlights KinCare's care coordination programs and other services that have a care coordination or case management component. Care coordination services can also be engaged by clients, family members, guardians, trustees or attorneys to assist in coordinating care and support services for clients.
