Home Care Packages

The type of support and health care we need at home is as individual as we are, and changes with our circumstances. 

Home Care Packages are available to people over 65 who need support to live at home.

After you have been assessed by your Aged Care Assessment Team (ACAT), you are ready to choose your care.  We can walk you through a range of care options and set goals that feel right for you.

There are six areas for care and support at home you can explore to develop a personalised program that meets your goals.  Our Quality of Life approach helps you design your care around your needs. KinCare will guide you to set goals to realise a better quality of life.

By asking questions about your quality of life, your consultant can ensure your plan best suits your needs, bringing you better health and well-being. For example:

  1. How would you like to keep connected to family and friends?
  2. What worries you most about your health and care?
  3. Which lifestyle and care choices will most maintain your feeling of independence?
  4. What achievements and day to day activities are most important you?
  5. What needs to be done for you to feel free from risk, danger and anxiety?
  6. What gives you a personal sense of comfort in life?
