Community Aged Care Packages
Community Aged Care Packages are available to older people with multiple support needs who require a moderate amount of weekly support. In some circumstances younger people with a disability may also be eligible for these packages.
Community Aged Care Packages are an Australian Government Initiative.

Who can refer? |

In regions in which the Aged Care Assessment Team manages a CACP waitlist for the region, KinCare prioritises ACAT referrals. In other regions KinCare accepts referrals from any source, but eligibility for the service is dependent on an ACAT assessment. |

What services are offered? |
|
Community aged care packages provide a variety of supports to people with multiple care needs. Services can include domestic assistance, personal care, respite, monitoring of medication, transport for appointments, shopping and bill-paying, light gardening and social activities. Service plans are developed in consultation with each client. Services are provided to meet assessed needs as determined by a KinCare Program Manager. Assessments consider client functional capacities and the needs of each client relative to other clients eligible for the service. |

How does the program work? |

An Aged Care Assessment Team determines a client’s eligibility for a CACP. |

A KinCare Program manager conducts a detailed assessment to assess each client’s specific needs. |

Clients participate in developing a care plan that will appropriately meet their needs within the scope of a CACP. | |

Special needs |
|
KinCare’s services are planned around the needs of each individual client. KinCare can provide services that meet special language, cultural, religious, dietary and other needs. |

What about client safety? |
|
All KinCare staff have received specialised training to provide in-home care. Staff wear a uniform and photograph identification. All staff employed by KinCare must undergo police and working with children checks. |

How much does it cost? |
|
A contribution is required towards the cost of providing this service. Fees are subject to a fee cap. Special Consideration is available for clients who are assessed as financially disadvantaged. |
